ABSTRACT

OBJECTIVE@#To explore the effectiveness of lobulated pedicled rectus abdominis myocutaneous flap to repair huge chest wall defect.@*METHODS@#Between June 2021 and June 2022, 14 patients with huge chest wall defects were treated with radical resection of the lesion and lobulated pedicled rectus abdominis myocutaneous flap transplantation for reconstruction of chest wall defects. The patients included 5 males and 9 females with an average age of 44.2 years (range, 32-57 years). The size of skin and soft tissue defect ranged from 20 cm×16 cm to 22 cm×22 cm. The bilateral pedicled rectus abdominis myocutaneous flaps in size of 26 cm×8 cm to 35 cm×14 cm were prepaired and cut into two skin paddles with basically equal area according to the actual defect size of the chest wall. After the lobulated pedicled rectus abdominis myocutaneous flap was transferred to the defect, there were two reshaping methods. The first method was that the skin paddle at the lower position and opposite side was unchanged, and the skin paddle at the effected side was rotated by 90° (7 cases). The second method was that the two skin paddles were rotated 90° respectively (7 cases). The donor site was sutured directly.@*RESULTS@#All 14 flaps survived successfully and the wound healed by first intention. The incisions at donor site healed by first intention. All patients were followed up 6-12 months (mean, 8.7 months). The appearance and texture of the flaps were satisfactory. Only linear scar was left at the donor site, and the appearance and activity of the abdominal wall were not affected. No local recurrence was found in all tumor patients, and distant metastasis occurred in 2 breast cancer patients (1 liver metastasis and 1 lung metastasis).@*CONCLUSION@#The lobulated pedicled rectus abdominis myocutaneous flap in repair of huge chest wall defect can ensure the safety of blood supply of the flap to the greatest extent, ensure the effective and full use of the flap tissue, and reduce postoperative complications.

Resumen Introducción: La úlcera por presión más frecuente es la sacra. Si compromete el hueso puede provocar osteomielitis por lo que requiere aseo quirúrgico y colgajo miocutáneo de gluteus maximus por deslizamiento en V-Y. Materiales y Método: En el servicio de cirugía plástica del hospital del Salvador entre 2011 y 2020 se han operado 82 pacientes con ulceras sacras grado 4 que requirieron cobertura con colgajo miocutáneo de gluteus maximus en V-Y. De ellas se analizaron los últimos 37 pacientes. Resultados De los 37 pacientes, 12 fueron mujeres, 25 hombres, 17 parapléjicos, 12 tetrapléjicos y 8 deambulaban. 6 pacientes presentaron COVID-19. 24 fueron bilaterales y 13 unilaterales. Los colgajos cicatrizaron bien en 30 pacientes. Las complicaciones fueron de 16% consistente en 2 hematomas, 2 dehiscencias, 2 celulitis y 1 seroma, todas resueltas sin problemas. Seguimiento de entre 3 y 6 meses. Discusión El tratamiento quirúrgico con colgajo miocutáneo de gluteus maximus en V-Y, aparte de aportar volumen para ocluir la úlcera, aporta irrigación excelente con oxígeno, nutrientes y antibióticos que aseguran una óptima cicatrización. Dependiendo del diámetro de la úlcera el colgajo puede ser uni o bilateral. Dado lo complejo del tratamiento, en general, el porcentaje de complicaciones de 16% se considera bajo. Conclusión: El tratamiento quirúrgico de las úlceras por presión sacras con colgajos miocutáneos deslizantes de gluteus maximus ha sido exitoso con buenos resultados quirúrgicos con buen flujo sanguíneo y buena evolución.


Introduction: Pressure sores are the result of the compression of soft tissues in the prominent bones areas, mainly in patients without movement. If the depth of the ulcer compromises the sacral bone, the treatment will be the gluteus maximus myocutaneous flap in V-Y. Materials and Method: In the plastic surgery service of the hospital del Salvador 82 patients with sacral pressure sores grade 4 were operated on between 2011 and 2020 with gluteus maximus myocutaneous V-Y flap. The last 37 patients were analyzed. Results: With this treatment the flaps were doing well in all cases with good blood supply. Complications: dehiscence: 2 patients, cellulitis: 2 patients, hematoma: 2 patients and seroma: 1 patient. The overall complication was 16%. Follow up between three and six months. Discussion: The most important part in pressure sores is their prevention. When the ulcer is in prominent parts of the body, the sore, could be in different grades of depth. The classification of them is in grades 1: erythema, 2: subcutaneous tissue. These two grades are solved with conservative treatment. When the ulcers are in grade 3 or 4, and with little ulcer in the skin but with damage of the deep plane, the treatment will be with surgery. In our casuistic the most frequent pressure sore is in the sacrum treated with gluteus maximus sliding myocutaneous flap in V-Y. With this treatment the flaps were doing well in all cases with good blood supply. The complications of 16% were considered low. Conclusion: The sliding gluteus Maximus myocutaneous flap in V-Y for treatment for sacral pressures sores have been successful for our patients.

ABSTRACT

O xantogranuloma juvenil (XGJ) é um tumor benigno e o mais comum do grupo das doenças histiocitárias proliferativas nãoLangerhans. Lesões; 2cm são consideradas XGJ gigantes, com relatos de lesões de até 18cm. Lesões oculopalpebrais podem necessitar de tratamento cirúrgico para controle de sintomas. Esse trabalho relata o caso de um menino de 8 anos que teve as 4 pálpebras acometidas por XGJ gigantes, além do terço médio. Ele foi submetido a 3 ressecções, sendo uma bastante profunda, necessitando enxerto de pele de espessura total diretamente sobre o músculo levantador da pálpebra superior. Posteriormente, 3 procedimentos de lipoenxertia foram realizados, atingindo resultado funcional e estético adequado, sem recorrência lesional.


Juvenile xanthogranuloma (JXG) is the most common benign tumor of the group of non-Langerhans histiocytic proliferative diseases. Lesions >2 cm are considered giant JXG, with reports of lesions of up to 18 cm. Oculopalpebral lesions may require surgical treatment to control symptoms. ABSTRACT

Úlceras de pressão são alterações da integridade da pele e tecidos subjacentes, causadas por pressão, mais usualmente sobre proeminências ósseas, especialmente em áreas desprovidas de sensibilidade, levando à necrose e ulceração. Dados da literatura internacional estimam que 3 a 14% dos pacientes hospitalizados desenvolvem úlceras de pressão. Descrevemos a correção simultânea de úlceras sacral e isquiática extensas em paciente paraplégico jovem utilizando retalho fasciomiocutâneo de glúteo máximo e de face posterior da coxa.


Pressure ulcers are alterations of the integrity of the skin and underlying tissues, caused by pressure, more commonly on bony prominences, especially in areas devoid of sensitivity, which lead to necrosis and ulceration. Data from the international literature estimate that 3­14% of hospitalized patients develop pressure ulcers. We herein describe the simultaneous correction of extensive sacral and ischial ulcers in a young paraplegic patient, using a gluteus maximus fasciomyocutaneous flap from the posterior aspect of the thigh.

ABSTRACT

Introdução: Câncer de mama localmente avançado é caracterizado pelos estádios clínicos IIIb ou IV e representam de 20 a 25% de todos os casos. A reconstrução dos defeitos é feita com retalhos musculocutâneos e fasciocutâneos, sendo os mais utilizados o latíssimo do dorso e o reto abdominal. O objetivo é avaliar resultados das reconstruções de parede torácica em câncer de mama localmente avançados com retalhos musculocutâneos e fasciocutâneos. Métodos: Estudo retrospectivo, observacional descritivo, em único centro. Variáveis estudadas: dimensões do defeito e do retalho, tipo de retalho utilizado para a reconstrução, metástases cutâneas e viscerais, evolução pós-operatória e complicações. Resultados: 11 pacientes, com média de idade de 49 anos, com o lado esquerdo mais acometido. O tipo tumoral mais encontrado foi o carcinoma ductal invasivo. Os retalhos realizados foram: 2 latíssimos do dorso com desenho VY (LDVY), 2 latíssimos do dorso associados a retalho toracoabdominal (LDVYTA), 4 verticais do músculo reto do abdome (VRAM) e 3 toracoabdominais (TA). A área média dos defeitos foi 421,72cm2 e a área média dos retalhos utilizados foi de 451cm2. A complicação mais frequente foi deiscência parcial da ferida operatória, presente em 7 pacientes. Da amostra, 6 pacientes atingiram êxito letal. VRAM foi o retalho que apresentou mais complicações. A sobrevida média para VRAM foi de 25,5 meses, para LDVY de 17 meses, TA de 17 meses e LDVYTA de 20,5 meses. Conclusão: Os retalhos musculocutâneos e fasciocutâneos são eficazes para a reconstrução da parede torácica após a ressecção de neoplasias mamárias localmente avançadas.


Introduction: Breast cancer is the most common cancer among women worldwide. Locally advanced breast cancer is characterized by clinical stage IIIb or IV and accounts for 20­25% of all cases. Defects are reconstructed using myocutaneous and fasciocutaneous flaps, primarily from the latissimus dorsi and rectus abdominis muscles. The objective is to evaluate the results of thoracic wall reconstructions in cases of locally advanced breast cancer using fasciocutaneous and myocutaneous flaps. Methods: This was a retrospective, observational, and descriptive single-center study. Variables studied included defect size and flap dimensions, myocutaneous flap type, presence of cutaneous and visceral metastasis, postoperative evolution, and complications. Results: We selected 11 patients with a mean age of 49 years; the left side was the most commonly affected. The most common tumor type was invasive ductal carcinoma. The flaps were made of latissimus dorsi VY (LDVY) in two patients, latissimus dorsi associated with thoracoabdominal flaps (LDVYTA) in two, vertical rectus abdominus myocutaneous flap (VRAM) in four, and thoracoabdominal flaps (TA) in three. The mean defect area was 421.72 cm2, while the mean flap area was 451 cm2. The most frequent complication was partial dehiscence (seven patients). Six patients achieved lethal exit. VRAM flaps presented more complications. The mean survival for VRAM was 25.5 months, LDVY was 17 months, TA was 17 months, LDVYTA was 20.5 months. Conclusion: Myocutaneous and fasciocutaneous flaps are effective for chest wall reconstruction after locally advanced breast cancer resection.

Introdução: No presente trabalho, relatamos um caso de reconstrução do terço proximal do úmero direito e reanimação do cotovelo de um paciente masculino, de 20 anos, vítima de acidente automobilístico, com necrose óssea de 5 cm no terço proximal do úmero e avulsão do bíceps braquial. Métodos: Utilizamos o retalho ósseo da escápula, associado ao retalho miocutâneo do grande dorsal, tendo como pedículo os vasos subescapulares. Resultados: O paciente teve excelente evolução no pós-operatório, apresentando-se, no pós-operatório de 2 meses, com consolidação óssea e iniciando a flexão do cotovelo. Conclusões: Perante a utilização do retalho descrito, concluímos que esta modalidade de retalho se insere no arsenal dos retalhos ósseos de maior segurança nas reconstruções ósseas em geral.


Introduction: We report a case of reconstruction of the proximal third of the right humerus and rehabilitation of the elbow in a 20-year-old male patient who was injured in an automobile accident and developed bone necrosis of 5 cm at the proximal third of the humerus and avulsion of the brachial biceps. Methods: A scapular bone flap was used, together with a latissimus dorsi myocutaneous flap, using subscapular vessels for the pedicle. Results: The patient had excellent postoperative course, presenting in the 2-month postoperative period with bone consolidation and initiation of elbow flexion. Conclusions: This flap modality is a safe and useful option for bone reconstruction.

ABSTRACT

La romboplastia de Limberg es una técnica quirúrgica útil en la reparación de defectos faciales. Se presenta un paciente de 55 años de edad, de procedencia rural que acude a consulta de cirugía de cabeza y cuello por presentar lesión persistente de tres meses de evolución. Esta se localizaba en la región temporo-frontal derecha, ulcerada y dolorosa y provocaba molestias físicas y estéticas. Se realizó resección quirúrgica y biopsia que informó carcinoma basocelular con bordes de sección libres de lesión y quedó un defecto circular de aproximadamente 8 cm de diámetro. El tamaño y localización de esta lesión en un área poco extensible hizo imposible el cierre borde a borde, fue necesario diseñar la triple plastia de Limberg que requiere de tres colgajos concéntricos de piel de las regiones del cuero cabelludo, preauricular y frontal, seguido del decolado, transposición y sutura de los mismos sin comprometer su irrigación. La triple plastia de Limberg demuestra su vigencia en la reparación del defecto temporo-frontal y posibilita el resultado oncoplástico esperado(AU)


Limbergs rhomboplasty is a useful surgical technique for facial defect repair. Here is a 55 years-old male patient from a rural area, who went to the neck and head surgery service for a persistent lesion occurred three months before. The lesion was located in the right temporofrontal region, in the form of an ulcer that causes a lot of pain and physical and esthetical discomfort. Surgical resection and biopsy were performed, and the latter showed a basal carcinoma whose borders were lesion-free and an 8 cm round defect remained. The size and location of such lesion in a barely extensible area made the border-to-border closure of the lesion impossible. It was then necessary to design Limbergs rhomboplasty that requires three concentric cutaneous flaps from the scalp, preauricular and frontal regions, followed by dissection, transposition and suture of such flaps without affecting blood irrigation. Limbergs rhomboplasy shows that it is a good technique for forehead defect repairs and facilitates the expected oncological and esthetic outcome(AU)

ABSTRACT

INTRODUÇÃO: As sequelas de mastectomia se apresentam de formas variadas exigindo, do cirurgião, experiência e criatividade para obter o melhor resultado na reconstrução de uma nova mama. Dentre as dificuldades para que este objetivo seja alcançado, está o desafio de um melhor preenchimento do polo superior da mama. O objetivo deste trabalho é apresentar uma opção terapêutica alternativa para correção do polo superior dentre as técnicas já existentes com este propósito. MÉTODO: A técnica cirúrgica utilizou o retalho de músculo grande dorsal com extensão gordurosa para preenchimento do polo superior da neomama durante sua reconstrução. A técnica descrita foi utilizada em 8 pacientes durante a reconstrução mamária tardia, com idades variando entre 39 e 70 anos. O tamanho desta extensão gordurosa variou entre 4,0 × 10,0 e 7,0 × 13,0 cm. O componente gorduroso do retalho foi avaliado após 3 meses através de ressonância magnética. RESULTADOS: Foram usados implantes mamários que variavam entre 270 e 435 ml. O acompanhamento pós operatório variou entre 3 meses a 1 ano. Ocorreram 2 casos de epidermólise (28%) na junção do retalho cutâneo com a área receptora. Não houve perda ou sofrimento do retalho. Os resultados demonstram clinicamente ou visualmente que a correção da depressão do polo superior da neomama foi alcançada adequadamente, bem como a viabilidade do retalho gorduroso, observada nos exames de imagem (ressonância magnética). CONCLUSÃO: A técnica proposta é uma alternativa adequada para o tratamento da maioria dos casos de reconstrução mamária em que se busca o preenchimento da depressão existente no polo superior da mama.


INTRODUCTION: The sequelae of mastectomy presents in numerous ways, requiring the experience and creativity of the surgeon to achieve the best result in the reconstruction of a new breast. One of the difficulties in achieving this objective is the challenge of adequately filling the upper pole of the breast. The objective of this work was to present an alternative therapeutic option for correction of the upper pole of the neobreast. METHODS: In our surgical technique, a latissimus dorsi muscle flap with fat extension is used for filling the upper pole of the neobreast during its reconstruction. The described technique was used in 8 patients during late breast reconstruction. The patients' ages ranged from 39 and 70 years. The size of the fat extension ranged from 4.0 × 10.0 cm to 7.0 × 13.0 cm. The fat component of the flap was evaluated after 3 months by using magnetic resonance imaging (MRI). RESULTS: Breast implants that varied in volume between 270 and 435 mL were used. The follow-up period after surgery ranged from 3 months to 1 year. Two patients had epidermolysis (28%) at the junction of the cutaneous flap and the receiver area. No graft loss or other complications occurred. Our results demonstrate clinically or visually that correction of the depression in the upper pole of the neobreast was satisfactorily achieved. The viability of the fat flap, as observed on MRI, was adequate. CONCLUSION: The proposed technique is a suitable alternative method for filling the depression in the upper pole of the neobreast in most cases of breast reconstruction.

ABSTRACT

INTRODUÇÃO: Uma grande evolução ocorreu desde o primeiro registro de transferência de tecido abdominal para reconstrução de mama pós-mastectomia. O retalho baseado em vasos perfurantes da artéria epigástrica inferior (DIEAP flap) apresenta-se com um dos mais recentes desenvolvimentos da área. MÉTODOS: Este artigo analisa fatos importantes na área de reconstrução autóloga da mama utilizando retalhos baseados no abdome, com ênfase nos retalhos microcirúrgicos vascularizados por pedículos perfurantes. RESULTADOS: Na experiência inicial do serviço, pudemos verificar que o retalho se comportou de acordo com a experiência relatada na literatura. CONCLUSÃO: O DIEAP flap apresenta uma possibilidade maior de escultura e ganho volumétrico na mama reconstruída, além de evolução pós-operatória muito positiva.


INTRODUCTION: Great advances have been reported since the first abdominal tissue transfer carried out for breast reconstruction after a mastectomy. The deep inferior epigastric artery perforator (DIEAP) flap is one of the most recent advances in this area. METHODS: In this article, we evaluate the important aspects in the field of autologous breast reconstruction with abdominal-based flaps, with emphasis on microsurgical flaps vascularized by perforating pedicles. RESULTS: During the initial experience of this procedure, we were able to verify that the flap behaved according to what was reported in the literature. CONCLUSION: The DIEAP flap provides a great degree of sculpting and volumetric gain to the reconstructed breast besides allowing a positive postoperative course for the patient.

ABSTRACT

INTRODUÇÃO: Revisão da literatura sobre o retalho miocutâneo inferior do músculo trapézio, apresentando suas vantagens, técnica cirúrgica e complicações. MÉTODOS: Estudo de 19 casos tratados no INCA, através de técnica específica, e comparação dos dados com a literatura internacional. RESULTADOS: Todas as reconstruções atingiram seus objetivos, com correção dos defeitos em partes moles e pele. O efeito estético foi considerado bom pelos pacientes e pela equipe médica. Foi observada uma taxa de 21% de complicações. Não houve necrose ou infecção nos retalhos. Em relação às funções, 89,5% apresentaram função motora preservada e 10,5%, déficit funcional por acometimento tumoral do nervo acessório. CONCLUSÃO: A despeito do aprimoramento contínuo das técnicas microcirúrgicas, os retalhos miocutâneos pediculados ainda encontram indicações nas reconstruções após ressecções oncológicas alargadas. O retalho miocutâneo inferior do músculo trapézio é uma alternativa segura e aplicável para as reconstruções de partes moles das regiões cervicais lateral e posterior, da região lateral da cabeça, da região do ombro e para as regiões paraespinhal superior e paraescapulares.


INTRODUCTION: In this article, we present a literature review on the lower myocutaneous trapezius flap, and report its advantages, surgical technique, and complications. METHODS: We studied 19 patients treated at INCA with a specific technique, and compared the outcomes with those reported in the international literature. RESULTS: All reconstructions achieved the procedural objectives, and the defects of soft tissues and skin were repaired. The aesthetic outcome was considered satisfactory by the patients and medical staff. The rate of complications was 21%. No necrosis or signs of infection were detected in the flaps. Concerning the functions, 89.5% of patients preserved their motor function, whereas reduced functional movements were observed in 10.5% of the patients due to tumor invasion to the accessory nerve. CONCLUSIONS: Despite the continuous advancements in microsurgical techniques, pedicle myocutaneous flaps are still indicated for reconstruction procedures after extended cancer resections. The use of the lower trapezius myocutaneous flap is safe and represents a suitable option for the reconstruction of soft tissues of the lateral and posterior cervical regions, side of the head, shoulder, and upper paravertebral and parascapular regions.

ABSTRACT

INTRODUÇÃO: A fístula palatina é a complicação mais frequente após palatoplastias e sua presença traz diversas implicações. Sua apresentação é diversificada e seu reparo pode ser difícil, o que se traduz na existência de uma diversidade de técnicas cirúrgicas descritas. OBJETIVO: Relatar a correção cirúrgica de fístula palatina anterior com retalho miomucoso labial superior, além de fazer uma breve revisão da literatura. RELATO DE CASO: Paciente submetido à correção de fissura palatina completa, apresentando, no pós-operatório mediato, fístula anterior de palato duro e processo alveolar, submetido à correção da fístula palatina oronasal com retalho miomucoso de lábio superior. O paciente evoluiu satisfatoriamente, sem complicações e sem recidiva da lesão após um ano de seguimento. CONCLUSÃO: A técnica em questão mostrou-se simples e eficiente, prestando-se à correção da fístula palatal anterior.


INTRODUCTION: Palatal fistula is the most common complication after palatoplasty, and its presence entails various implications. Its presentation is diverse and repair can be difficult, which is reflected in the existence of a numerous surgical techniques described for its treatment. OBJECTIVE: To report the surgical correction of palatal fistula with a myomucosal upper lip flap, along with a brief review of the literature. CASE REPORT: The patient underwent repair of complete cleft palate. At the immediate postoperative time, an anterior fistula of the hard palate and alveolar process was present and was subjected to a correction with an oronasal myomucosal upper lip flap. The patient progressed satisfactorily, without complications or lesion recurrence after 1 year of follow-up. CONCLUSION: The technique presented is a simple and efficient method for correction of an anterior palatal fistula.

ABSTRACT

INTRODUÇÃO: Reconstruções de defeitos complexos na face constituem um desafio para os cirurgiões. Após ressecção da lesão, o reparo nasal é difícil, já que possui uma variedade de tecidos e área local doadora reduzida para reconstrução. Já as lesões de lábio possuem como principal dificuldade a restauração da função, priorizando a integridade da estrutura esfincteriana, a continência oral, a abertura bucal e a sensibilidade. Entre as alternativas para reconstrução dessas áreas, podemos optar por retalhos locais, entre os quais o nasogeniano. OBJETIVO: Analisar desfechos de reconstrução nasal e de lábio superior em dois tempos cirúrgicos, utilizando retalho nasogeniano, após defeito complexo causado por ressecção de carcinoma espinocelular (CEC). RELATO DO CASO: Paciente do gênero feminino, 56 anos, apresentou CEC de progressão avançada em região nasal da columela, terço anterior do septo e parte do lábio superior. A paciente foi submetida à reconstrução com retalho nasogeniano e, em segundo momento cirúrgico, ao enxerto de cartilagem costal para reconstrução de columela. Apresenta evolução com boa integração e viabilidade dos enxertos e retalhos. O resultado estético foi satisfatório. CONCLUSÃO: A utilização de retalho nasogeniano como opção terapêutica para a reconstrução parcial de nariz e de lábio superior apresentou desfechos funcionais/estéticos favoráveis. Além disso, a segmentação do procedimento cirúrgico traz segurança na utilização dos retalhos locais, principalmente em indivíduos de difícil cicatrização.


INTRODUCTION: Reconstruction of complex facial defects is a challenge for surgeons. After the excision of a lesion, nasal reconstruction is challenging because of the variety of tissues involved and the reduced local donor area for reconstruction. On the other hand, a major difficulty in the reconstruction of lip lesions is restoration of function, with the priority being the maintenance of the integrity of the sphincter structure, oral continence, mouth opening, and sensitivity. Among the alternatives for the reconstruction of these areas are local flaps, including the nasolabial flap. AIM: To analyze the outcome of nasal and upper lip reconstruction performed in two surgical stages, by using a nasolabial flap for complex defects resulting from the resection of squamous cell carcinoma (SCC). CASE REPORT: A 56-year-old female patient presented with advanced SCC in the nasal region of the columella, anterior third of the septum, and part of the upper lip. She underwent reconstruction with a nasolabial flap and, in a second surgical procedure, with a costal cartilage graft for the reconstruction of the columella. The procedures resulted in good integration and viability of grafts and flaps. The aesthetic result was satisfactory. CONCLUSION: The use of a nasolabial flap as a therapeutic option for the partial reconstruction of the nose and upper lip produces favorable functional and aesthetic outcomes. In addition, the division of the surgical procedure into stages provides safety in the use of local flaps, particularly in patients with poor healing.

ABSTRACT

INTRODUÇÃO: A reparação da parede abdominal após reconstrução mamária com retalho TRAM representa um desafio para o cirurgião, ainda sem consenso na literatura em relação à melhor técnica. O objetivo deste estudo foi avaliar a eficiência da tela Ultrapro® em dois planos anatômicos distintos para reparação da parede abdominal pós-retalho TRAM. MÉTODO: Um estudo retrospectivo foi realizado por meio da revisão de prontuários de 24 pacientes submetidas à reconstrução de mama com retalho TRAM pediculado e reparo da área doadora abdominal com tela dupla de polipropileno e poliglecaprone - Ultrapro® pela Divisão de Cirurgia Plástica do HCFMRP-USP. Foram avaliados fatores de risco para hérnias ou abaulamentos abdominais, momento da reconstrução de mama; complicações pós-operatórias, incluindo hérnias ou abaulamentos abdominais, e tempo de seguimento pós-operatório. RESULTADOS: Do total de 24 pacientes com idade média de 51 anos, 10 (41,6%) apresentavam alguma comorbidade. Em 95,8% das pacientes a reconstrução mamária foi tardia e o retalho TRAM foi unipediculado em 58,4% dos casos. As complicações pós-operatórias mais frequentes foram deiscência de sutura (25%) e seroma (21%). Duas pacientes (8,4%) tiveram diagnóstico de hérnia abdominal e três pacientes (12,5%) apresentaram abaulamento abdominal. O tempo de seguimento pós-operatório variou de 5 a 48 meses (média 23,4 meses, DP: 13,28). CONCLUSÃO: O uso da tela híbrida Ultrapro® em dois planos anatômicos demonstrou ser mais uma alternativa para o reparo da parede abdominal pós retalho TRAM em reconstrução mamária, com baixa morbidade da área doadora abdominal e índices de complicações semelhantes aos dados da literatura.


INTRODUCTION: The repair of the abdominal wall after breast reconstruction with a transverse rectus myocutaneous (TRAM) flap is a challenge for the surgeon, and there is still no consensus in the literature about which is the best technique. The objective of this study is to evaluate the efficiency of the Ultrapro® mesh in two different anatomical planes for the repair of the abdominal wall after TRAM flap surgery. METHOD: This is a retrospective study conducted through a medical records review of 24 patients who underwent breast reconstruction with a pedicle TRAM flap, and repair of abdominal donor site with a dual mesh of polypropylene and polyglecaprone - Ultrapro, at the Plastic Surgery Division of the Clinics Hospital of the Medicine Faculty of Ribeirão Preto - University of São Paulo. We evaluated the risk factors for abdominal hernias or bulges, time of breast reconstruction, postoperative complications (including abdominal hernias or bulges), and postoperative follow-up. RESULTS: Of the 24 patients with a mean age of 51 years, 10 (41.6%) had a comorbidity. In 95.8% of the patients, breast reconstruction was late; the TRAM flap was a single pedicle in 58.4% of cases. The most frequent postoperative complications were suture dehiscence (25%) and seroma (21%). Two patients (8.4%) were found to have abdominal hernia, and three patients (12.5%) had abdominal bulging. The postoperative follow-up ranged from 5 to 48 months (average, 23.4 months, SD = 13.28). CONCLUSION: The use of the Ultrapro hybrid mesh at two anatomical planes proved to be an alternative for the repair of the abdominal wall after TRAM flap surgery for breast reconstruction, with low morbidity of the abdominal donor site and complication rates similar to literature data.

Background: Detection of breast cancer coupled with mastectomy negatively affects patients' emotional state. The psychosocial effect is a distortion of body image, including decreased femininity, sexuality, and quality of life. Breast reconstruction can be performed at different times using different techniques, improving the comfort and quality of life of these patients. Method: This was a retrospective study of 16 female patients who underwent breast reconstruction between October 2008 and October 2011. The following parameters were analyzed: age, complications, techniques, time of reconstruction, adjuvant therapies(i.e., chemotherapy and radiotherapy), and satisfaction with reconstruction. Data were collected through chart reviews, and patient satisfaction with the procedure was measured using a specific questionnaire. Results: Among the 16 patients, 15 (93.75%) reported being very satisfied, and the remaining 1 patient (6.25%) was somewhat satisfied; no patient reported dissatisfaction after reconstruction; the 1 patient who reported being somewhat dissatisfied had not completed all stages of breast reconstruction. Conclusions: Despite the small sample size, the results indicate that reconstruction positively affected the quality of life of patients undergoing mastectomy, regardless of the technique or time chosen for reconstruction, resulting in a high degree of satisfaction with a change in body contouring.


Introdução: A descoberta do câncer de mama, aliada à mastectomia, impacta negativamente no estado emocional da paciente, cujo efeito psicossocial é uma distorção da imagem corporal, com piora na feminilidade, sexualidade e qualidade de vida. A reconstrução mamária pode ser realizada em diferentes tempos e por meio de diversas técnicas, trazendo conforto e melhora da qualidade de vida dessas pacientes. Método: Foi realizado estudo retrospectivo de 16 pacientes do gênero feminino submetidas a reconstrução de mama no período entre outubro de 2008 e outubro de 2011. Foram observados os seguintes parâmetros: idade, presença de complicações, técnicas, tempo de reconstrução, terapias adjuvantes (quimioterapia e radioterapia) e grau de satisfação com a reconstrução. Os dados foram levantados por meio de revisão de prontuário e da satisfação das pacientes com o procedimento mensurada por questionário específico. Resultados: As pacientes muito satisfeitas representaram 93,75% dos casos (15 pacientes) e as 6,25% restantes (1 paciente) apresentaram-se pouco satisfeitas. Nenhuma paciente relatou insatisfação após a reconstrução. A paciente que relatou estar pouco satisfeita ainda não concluiu todos os estágios da reconstrução mamária. Conclusões: Apesar da pequena amostra de pacientes, a reconstrução impacta positivamente na qualidade de vida das pacientes submetidas a mastectomia, independentemente da técnica e do tempo escolhido para essa reconstrução, trazendo elevado grau de satisfação com a alteração do contorno corporal.

ABSTRACT

São apresentados 12 (doze) casos clínico-cirúrgicos, em que foram realizados retalhos fasciosubcutâneos para reconstruir perdas de substância no terço inferior de perna causadas por diversas patologias. A experiência é descrita e discutida considerando-se a técnica cirúrgica, as vantagens e as desvantagens do procedimento. Esta técnica representa mais uma opção, de fácil execução e baixa morbidade, no arsenal do cirurgião plástico.
